Asthma is a heterogeneous disease that can be deceptively difficult to diagnose. In the absence of objective testing, patient
history and symptoms such as wheezing and cough may not be definitive clinical markers of asthma. Further, other causes of
airway obstruction leading to wheezing include conditions such as COPD (e.g., chronic bronchitis or emphysema), congestive
heart failure, pulmonary embolism, mechanical obstruction of the airways (benign and malignant tumors), pulmonary infiltration
with eosinophilia, cough secondary to drugs (e.g., angiotensin-converting enzyme (ACE) inhibitors), or vocal cord dysfunction.2
Lastly, methacholine challenge tests may also be helpful in diagnosing cough variant asthma (a chronic cough which is the
principal — or only — manifestation of asthma) especially in young children.
